The law requires that a public information request be in writing. The following form is for your convenience and may be completed using Adobe Acrobat Reader. Once completed, the form may be emailed or printed and either mailed, faxed or submitted in person to the City Secretary’s Office. A button for email submission has been provided at the bottom of the form, simply press it and follow the prompts.
Users should download the form, complete it in Adobe Acrobat Reader, save, and then submit the form.

All requests for public information are treated uniformly. Once the request is submitted, it will be reviewed to determine if the information is deemed public. If the information is public, then the information will be processed and sent out in a timely manner. The Office of the City Attorney determines whether or not the requested information can be released. A response to the request will be issued within 10 business days.
